How to Schedule an Appointment

1. Call our office at 480-994-5225 OR fill out our online form and indicate you are interested in a virtual appointment.
2. Our team will call to confirm your appointment and will send you an email invitation for the call.
3. If you are a new patient, please fill out our new patient registration form online .

*Important, please note: There is a $40.00 fee to schedule a virtual appointment.

If you have insurance, it is billed once the patient is seen in the office. Any insurance monies paid can be applied to account, put towards future care or refunded. (The second/follow-up exam in the office is included for no additional cost.)

**Our staff will review this information with you before you are sent the invitation link for the appointment.
