Please feel free to complete our new patient paperwork packet online, or if you wish, print it out, complete it and bring it to your first appointment. If you would prefer to complete it here in our office, please allow an additional 10-15 minutes prior to your set appointment time.
If you are a new patient that has dental insurance, typically the examination and any necessary films are a covered benefit (often coverage is at 100% but not always). If you do not have any dental insurance coverage, this new patient special is available for you. We want everyone to be able to come in for an assessment.
A new patient will receive a complete digital film "x-ray" series, gum check assessment, and full exam that includes intraoral and or digital photos.
